X12 Forms |

Changes to transaction sets should be submitted to the UIG Guidelines Committee. The change requests should be submitted on the ASC Xl2 Work Request Form. The change requests will be reviewed by the UIG Standards and Maintenance Commitee. The submitter will be contacted if questions arise concerning the request. Work Request Forms will be submitted to ASC X12 by the UIG Standards and Maintenance Committee. All sections of the Work Request Form must be completed. A complete explanation of the business need (Section C. Reason for Change) is critical if the change is to be approved by ASC Xl2 Any request for changes involving new data elements or data element codes which reference a code list published by an external (non-Xl2) organization requires the submission of ASC X12 Form For New Or Revised Appendix A Reference with the Work Request Form. |

Standards) The latest and prior version/release containing all approved or draft standards of Transaction Sets plus segment Directory (Xl2.22), Data Element Dictionary (Xl2.3) and Code Sources. The Xl2 Standards publication will normally contain the Interchange Control Structure (Xl2.5) and Application Control Structure (Xl2.6). |

| X12.5 Interchange Control Structure Defines control structures for the electronic interchange of business transactions. Provides the interchange envelope of a header and trailer for the electronic interchange through a data transmission and a structure to acknowledge the receipt and processing of this envelope. |

| X12.6 Application Control Structure Defines the basic structure, content, and syntax of business transactions for computer to computer interchange. |
